What is FormLizard?

FormLizard is a web application that turns forms into online interviews, then takes the interview answers and fills out the forms. It is comparable in how it works to today’s popular tax software, except it uses your forms.

By converting forms into online interviews, end-users can simply type their answers to clearly stated questions in the space provided. When they finish, they can select the completed form or forms and download for printing.

What is it used for?

FormLizard can be used for many types of forms and documents, from school registration forms, to court filings to complex contracts, even repetitive reports. If you find yourself having to fill-out paperwork, you would probably benefit by using FormLizard. Also, if you are responsible for ensuring the right forms, contracts or other documents are used, you will find FormLizard to be a great help.

What industries can use FormLizard?

Although the promise of the paperless office has been around for decades, the reality is that most business processes contain a lot of paper. FormLizard is a great tool for medical providers, K-12 schools, colleges and universities, courts, financial institutions, insurance agents, state and local governments, sales organizations, procurement organizations, HR departments, and more. If it's information to be captured on paper, FormLizard is the tool.

Data Security and Data Storage

Is my data safe?

We use some of the latest tools and techniques to ensure the integrity and safety of your data. From the uniqueness of our data design to the specific operational controls, we seek to provide a safe and secure environment for a diverse collection of our customers’ data. Some of the measures we take include the following:

All data is encrypted while being transmitted. That is, all plans (even the free-trial) includes 256 bit SSL encryption, so that the data moving from the server to your computer is transmitted in an encrypted manner.

Similarly, all customer data that resides on our servers is also encrypted. This means that the data is not readable without the correct 256 bit Advanced Encryption Standard key.

Is FormLizard HIPAA compliant?

It is up to each organization to determine the security requirements they need. To assist our clients in determining if our security is appropriate for their needs, we have constructed the following table of the HIPPA recommendations, both required and addressable, and describe our implementation.

Access Controls:

All accounts are password protected with minimum medium strength passwords. Access is also available through Google and Google Apps SecurID token-based two-legged authorization. Account manager can establish privilege for users including solely completing interviews or completing interviews and configuring forms and interviews. Account managers can terminate sessions in progress with a single click. Passwords are not stored in browsers and are always encrypted on the servers preventing anyone from seeing the password.

All users have a unique identifier to use as their login which is their verified email address.

FormLizard is available from any Internet connected computer in the event it must be accessed to retrieve information by authorized users during an emergency.

Encryption and Decryption:
All data being transmitted is encrypted via 256 bit SSL encryption.

All data at rest (while stored) is encrypted via 256 bit Advanced Encryption Standard.

Audit Controls:
All connected sessions are automatically logged including transaction information and end-users IP address. Our system administrators have access to audit logs and can provide them to Account Holders upon verified request.

Integrity:
Access to the application servers is limited to senior Blue Burro managers. Data integrity is regularly tested and checked to verify information has not been altered.

FormLizard is hosted on dedicated, virtual servers offered via Media Temple in the US. Media Temple (dv) servers provide dedicated processor cycles, dedicated memory and dedicated storage. The dedicated servers ensure no data is shared among other servers. Using Parallels Virtuozzo Containers, we monitor the server's performance with real-time resource statistics including memory, cpu, and network utilization. The facility infrastructure includes: N+2 redundancy backup power, N+2 redundant cooling system, 24/7 monitored fire suppression, and 24/7 armed security. You can learn more at mediatemple.com.

Do you store my credit card information?

No, we do not store your credit card information. Credit card information is handled via a national PCI compliant processor.

How many completed forms can I store?

All of our plans allow you to store an unlimited number of completed forms.

How long is my data stored?

We store your data for as long as you have an active account, plus 60 days. In the event you want your data destroyed immediately, please contact us via email at support@formlizard.com, using the email account for the account holder, and including a phone number we can use to verify identify of the organization.Upon verification, we will erase all of the account data.

Is my data backed up?

Yes, all data is backed up daily.

End-User Access Options

Can I make my FormLizard accessible only to those whom I want to have access?

Yes, the account owner can select whether their instance of FormLizard will accept new user registrations from the end-user login page, or not. If not, then only the account owner and administrators can create new accounts for end-users.

Can I make my FormLizard forms accessible to the public?

Yes, the account owner can select whether their instance of FormLizard will accept new user registrations from the end-user login page. If so, then end-users can navigate to the login page, and they will see a registration box and link to start the registration process.

If I make my FormLizard accessible to the public, how does the public access FormLizard?

As a web application, anyone knowing the URL to a FormLizard instance can see the login page. However, only those with valid login credentials can access the site as an end-user or an administrator.

Preparing Your Forms

How do I prepare my forms for uploading?

What formats can by forms be in for uploading?

FormLizard accepts RTF and TXT formats for uploading. These are common formats that most word processors can export. We recommend using Microsoft Work or Google Docs to create the RTF or TXT files.

How do I save my form as an RFT document?

This depends upon the word processing software being used. Typically, the selection is available under menu selection File, then look for Save As. Often on the panel that appears, you can select the file type to be saved. From the list the word processor displays, choose RTF.

Fields and Tags

Can I delete fields?

No. Deleting fields is not allowed. However, just rename and re-use any fields. If you have extra fields and want them to be at the end of the list of fields, just label them as zzfield1, zzfield2, or any other name that will make them sort to the end of the alphabetically sorted list of fields names.
