Creating Conditionals
What's a conditional field

- Conditional fields trigger an extra form if answered yes. For example, the above form asks if the student needs to take any medications at school. If the answer is yes, a Medication Authorization form is to be filled out.
To make the field a conditional, we just set the type to "Conditional" in the edit field window

Set the "Yes" action

- Next we need to pick which form will be filled out if the user answers "Yes." In this case we want the user to fill out the Medication Authorization Form which we have already tagged and uploaded to FormLizard. Now when a user selects yes, they will be presented with the questions from the Medication Authorization Form and their answers will be inserted into the form.
Set the "No" action

- You can also set a message to appear if the user clicks no or you can leave it blank. In this case, we have decided to insert a message that the student does not need medication.
- Remember to click Save when you are finished.

- As you can see, more questions become available when answered yes
