Preparing Forms for Uploading into FormLizard


Open your document in a word processor

For best results, start with forms in Microsoft Word format. If your forms are in pdf, see if you can find the original MS Word document as this will save you some time.


Add FormLizard Tags to your document

-Anywhere a person would normally fill out information add a meaningful title set between double square brackets.
-For example where you would normally write the student's last name we have added a tag [[student last]]
-It doesn't matter what the tag is called as long as it is meaningful to you.


For sections that are multiple choice, only use one tag

We have condensed all of the multiple choice answers down to one tag

Finish tagging the rest of your form anywhere an answer is needed

Save in Rich Text Format (.rtf)

-When you save, there should be a drop down menu called "Save as type" or "Format". Click the drop down menu and select "Rich Text Format."
-Save.
